The headlines tend to focus on the newest glucagon-like peptide-1 (GLP-1) medications. But older GLP-1s still deliver solid results and might be the best fit for some people.

Back in 2014, liraglutide was the first GLP-1 medication approved for weight management in the U.S. A decade later, tirzepatide has built on its path, almost tripling liraglutide’s weight loss results in clinical trials with a dual-receptor mechanism. But more weight loss doesn’t always mean better.

Liraglutide (sold under the brand names Saxenda® and Victoza®) was one of the earliest GLP-1s in the market, and it was the first one approved for weight management. It activates the GLP-1 pathway with a half-life of roughly 13 hours. That means you have to inject it every day. 

Given the success of GLP-1s, researchers have worked hard to create longer-lasting and more effective GLP-1s. Tirzepatide (active ingredient in Zepbound® and Mounjaro®) is the result of that process. In addition to the GLP-1 pathway, tirzepatide activates the receptors of a second hormone called glucose-dependent insulinotropic polypeptide (GIP). GIP makes your cells respond to insulin more effectively and influences how your body processes and stores energy as fat.

Are tirzepatide and liraglutide effective for weight loss?

Both tirzepatide and liraglutide produce meaningful weight loss when you combine them with a reduced-calorie diet and exercise. But when it comes to effectiveness, tirzepatide stands out.

Participants at the highest dose of tirzepatide in the SURMOUNT-1 trial lost an average of 20.9% of their body weight over 72 weeks. Even the lowest dose, 5 mg, produced a 15% of average loss. Around 9 in 10 participants lost at least 5%, and more than half of people at the highest dose lost 20% or more.

Clinical evidence for liraglutide weight loss comes from the SCALE Obesity and Prediabetes trial. Adults taking liraglutide daily lost an average of 7.4% of their body weight over 56 weeks, compared to 3% with placebo. About 62% of participants lost at least 5% of their starting weight, and roughly a third lost 10% or more. 

In both cases, weight loss usually began during the first weeks of treatment and built throughout the course of the trials—though weight plateaus are common and don’t necessarily mean that the medication stopped working. Individual results varied according to dose, the person’s lifestyle, and each body’s reaction to the medication.

The trials involved different populations, timelines, and study design, so these numbers don’t work as a head-to-head comparison. But the pattern holds across indirect comparisons: Tirzepatide’s dual-pathway mechanism produces two to three times the weight loss of liraglutide’s single pathway.

Side effects and safety of tirzepatide vs. liraglutide

Due to the delayed digestion mechanism these two medications share, their most common side effects are gastrointestinal (GI). These reactions tend to be worse during dose escalation, while your body is adjusting, and fade over time. These reactions, however, happen more often with liraglutide than with tirzepatide:

Both medications carry some serious warnings on their labels:

  • Thyroid C-cell tumors: In animal studies, both drugs caused thyroid tumors in rodents. We still don’t know if this is the case for humans as well. But as a prevention measure, these medications are off the table for anyone with a personal or family history of medullary thyroid carcinoma or a rare genetic mutation called MEN2.
  • Pancreatitis: As both medications act on the pancreas to regulate insulin, there’s a small risk of pancreatic inflammation. In trials, it amounted to 0.3% with liraglutide and 0.2% with tirzepatide.
  • Gallbladder disease: Your liver releases more cholesterol into bile when you lose weight quickly. This cholesterol can crystallize into gallstones, generating gallstone inflammation. 
  • Acute kidney injury: The nausea, vomiting, and diarrhea these medications cause can lead to dehydration, which reduces blood flow to the kidneys and impairs their function.
  • Heart rate: Both liraglutide and tirzepatide can raise your resting heart rate slightly. Liraglutide's effect is more pronounced, and its label requires regular monitoring.
