HRT works by supplementing estrogen and sometimes progesterone. Progesterone is a sleep-supporting hormone that naturally declines in menopause. Dedicated sleep medications like hypnotics and sedatives target specific brain receptors to help you fall asleep or stay asleep regardless of hormones.

Why does menopause disrupt sleep?

A 2023 meta-analysis in Sleep and Breathing found that about half of postmenopausal women live with a sleep disorder. That number isn’t much lower for perimenopausal women. The reasons are layered but often trace back to the following main sources.

Direct hormonal shifts

Progesterone acts on your GABA receptors, which are part of the brain’s calming system. At the same time, estrogen works in areas of the brain that regulate your core body temperature (the hypothalamus) and your sensitivity to melatonin, a hormone that influences your sleep-wake cycle (circadian rhythm).   • Vasomotor symptoms (VMS): Approximately 80% of menopausal women experience VMS, including hot flashes and night sweats. When fluctuating estrogen levels destabilize your body’s internal thermostat, the brain misinterprets minor temperature changes as overheating and triggers a sudden cooling response. This response jolts your central nervous system awake to cool you back down.
  • Anxiety: Over 20% of menopausal women also experience heightened anxiety. As progesterone levels decrease, you lose their natural calming effects, leaving you more sensitive to stress and racing thoughts
  • Nocturia: More than 20% of menopausal women experience a frequent need to urinate at night. This is because estrogen supports the structural integrity of the pelvic floor and urethra. When levels decline, tissues can become thinner and less elastic. This often leads to increased frequency or urgency that forces you awake at night.

Does progesterone help you sleep?

Your body converts progesterone into a compound called allopregnanolone. This compound acts on GABA-A receptors, which are part of the brain’s calming system. By binding to these receptors, allopregnanolone promotes a natural sedative effect.

During menopause, progesterone levels fluctuate and decline, which can weaken allopregnanolone’s calming signal. For many, HRT helps restore these levels. A 2021 systematic review in The Journal of Clinical Endocrinology & Metabolism supports this. Researchers found oral micronized progesterone helped postmenopausal women fall asleep faster, likely due to this restorative calming effect. Crucially, oral progesterone also increases slow-wave (deep) sleep and reduces nighttime awakenings. It does so without the dependency, cognitive impairment, or rebound insomnia associated with conventional sleep medications.

Beyond its sedative properties, progesterone may also support sleep by acting on the hypothalamus. Researchers suggest it may decrease brain inflammation and help keep your internal thermostat steady, which can potentially cut down on hot flashes.

How does combined HRT support sleep?

Combed estrogen-progesterone therapy (EPT) can offer significant sleep benefits, building on all the ways progesterone alone supports rest. Adding estrogen addresses broader hormonal sleep disruptors.

By restoring levels with HRT, estrogen reduces sensitivity in the hypothalamus. This makes night sweats less likely to occur and pull you out of a deep sleep. The FDA approves estrogen therapy for this purpose, and the American College of Obstetricians & Gynecologists and the Menopause Society also recognize this benefit.

Beyond VMS, HRT may benefit sleep through other pathways, from supporting mood to easing joint pain. Because restoration of the hormonal environment causes these benefits, HRT warrants a conversation with your doctor.

How do common sleep medications compare?

HRT addresses hormonal drivers of poor sleep (the root cause), while sleep medications treat the symptoms (the effect). Common sleep medications include:

  • Benzodiazepines like Restoril®, Halcion®, and ProSom® cause a calming effect by targeting GABA-A receptors. While effective at helping you fall and stay asleep, they can cause tolerance or physical dependence. This makes them less ideal for long-term insomnia management.
  • Non-benzodiazepine hypnotics (Z-drugs) like Ambien® and Lunesta® also act on GABA-A receptors, but in a more selective way. Unlike benzodiazepines, which act like a master key across a broad range of receptors, Z-drugs target only specific receptors responsible for sleep.   • Dual orexin receptor antagonists (DORAs) like Dayvigo®, Quuviviq®, and Belsomra® block orexin, a wakefulness-promoting neurotransmitter. By turning off the brain’s alertness signal, they can help reduce insomnia. Potential side effects include next-day drowsiness and headache.

Because these medications treat the symptoms, they don’t necessarily fix the root hormonal cause of menopause-related insomnia. However, they can be useful tools in addition to HRT for symptom management. When is HRT (progesterone) the right path?

  • Timing: Research suggests that the benefits of hormone therapy are most pronounced when you start before age 60 or within 10 years of menopause’s onset. Your specific stage in the menopausal transition influences whether this intervention is appropriate. 
  • Candidacy: HRT isn’t right for everyone. Your doctor will conduct a thorough risk assessment based on your medical history. This will include any predispositions to conditions that HRT might exacerbate, like certain types of cancer or cardiovascular history. They’ll weigh personal risk factors before prescribing. 

Can you take HRT and sleep medications at the same time?

You can typically take HRT and most common sleep medications at the same time. There are no severe side effects from taking HRT with benzodiazepines, over-the-counter antihistamines, melatonin, or DORAs.
