If early signs of perimenopause have started to affect your life, your provider may bring up hormone replacement therapy (HRT). Two common treatment options tend to come up: Premarin® and Estrace®. Both of these estrogen therapies treat menopause symptoms and produce comparable relief. 

But that’s where their similarities end. Their active ingredients come from distinct sources and behave differently in your body. This article breaks down Premarin® versus Estrace®, the differences between them, and what to consider as you and your provider decide which one fits your situation.

What are the main differences between Estrace® and Premarin®?

Premarin® and Estrace® are both FDA-approved estrogen therapies for menopause, but they come from different sources:

• Premarin® contains conjugated estrogens, a mixture of more than 10 estrogens extracted from pregnant mares’ urine.
• Estrace® contains estradiol, a plant-derived bioidentical estrogen that’s structurally identical to what your ovaries naturally produce. 

Some people claim that bioidentical hormones are safer or more effective than other treatment types, but there’s no solid evidence to support this claim.

Who should take Estrace® vs. Premarin®?

Neither medication is automatically the better choice. The North American Menopause Society’s (NAMS) 2022 position statement recommends shared decision-making between you and your provider, based on your symptoms, medical history, and preference. Here are the factors that typically guide that conversation.

A provider might recommend Estrace® when:

• You have blood clot risk factors: A personal history of blot clots, obesity, or a clotting disorder puts you at a higher risk of venous thromboembolism (VTE). Oral estrogen treatments are linked to higher clotting factors because they travel through the liver before reaching the rest of the body. This high concentration of estrogen in the liver stimulates cells to ramp up production of clotting factors. Transdermal Estrace® (patches, gels, sprays) bypass the liver processing linked to higher clotting factor levels because they absorb directly into the tissues.
• You plan long-term hormone therapy: Estradiol offers greater long-term metabolic compatibility because it breaks down through the same pathways your body has always used.

A provider might recommend Premarin® when:

• Estradiol failed to relieve your symptoms: Premarin® activates estrogen receptors in a different pattern than estradiol alone, and some women respond better to this alternative pathway.
• Cost was previously an issue: Generic conjugated estrogen tablets now cost significantly less than brand-name Premarin®. If cost was previously an issue, revisiting the generic option may be more viable at this point.

Estrace® vs. Premarin® efficacy

Despite their distinct molecular structures, both Estrace® and Premarin® perform equally well at menopausal symptom relief. The 2016 Cochrane review of 23 trials found no evidence of a difference in effectiveness between estradiol and conjugated estrogens for hot flashes. In the one direct oral comparison, both estradiol and conjugated estrogens reduced hot flash frequency by roughly 80–91% from baseline, with no statistically significant difference between them.

Safety outcomes

While effectiveness is virtually identical, Premarin® and Estrace’s® safety profiles differ when looking at VTE. The ultimate safety advantage comes from choosing a transdermal delivery route over a pill, and only Estrace® is available in transdermal formats. Here’s the data.

Oral pill vs. oral pill

A 2023 claims-based study of over 20,000 blood clot cases found that within oral formulations, conjugated estrogen combinations were about 33% more likely to cause blood clots than estradiol combinations. The reason for this is that any estrogen taken by mouth travels through the liver at high concentrations before reaching the rest of your body. That concentrated liver exposure drives up production of clotting factors, inflammatory markers like C-reactive protein, and fats (triglycerides).

Oral pill vs. transdermal

But oral versus transdermal shows the largest gap. The ESTHER study found oral estrogen users had 4.2 times the blood clot risk of nonusers, while transdermal estrogen users had no elevated risk at all. Transdermal estradiol—patches, gels, sprays—enter your bloodstream directly through the skin, so the liver never sees that concentrated surge. In 2013, ACOG confirmed that transdermal estrogen “has little to no effect in elevating prothrombotic substances.”

Cost and access

When factoring cost and insurance coverage into your HRT choice, availability differs significantly depending on whether you’re using an oral pill or localized treatment:

• Oral estradiol: Generic estradiol tablets often cost between $9–$40 for a 30-day supply without insurance. Because generic estradiol has been around since 1997, doctors rarely prescribe brand-name Estrace®. Generic patches and topical creams are also widely accessible, costing $30–$183 per month.
• Oral Premarin®: For over 80 years, brand-name Premarin® tablets ran $215–$285 per month out of pocket due to the lack of generic alternatives. However, that changed when generic conjugated estrogen tablets launched in November 2025. Early pricing puts the generic at roughly $60–$150 per month. 
• Topical and local options: There’s still no generic version of Premarin Vaginal Cream®, which still costs $250–$590 per tube. Women who rely on this vaginal estrogen cream don’t yet have a lower-cost alternative.
