PRODUCT LINE DESCRIPTIONS
All available Product Lines are described below. Specific Product Lines actually being purchased by Customer are identified on the Order Form’s Fee Schedule.
PRODUCT LINE
Maternity
- Up to 21-month end-to-end program supporting expecting mothers and fathers during pregnancy and for 12 months after birth through the postpartum and return-to-work periods
- Program includes Pregnancy, Partner, and Loss tracks, and a Participant may move from one track to another within the same Maternity Product Line enrollment up to 12 months postpartum
- 61-week integrated return-to-work curriculum and professional coaches introduced in first trimester
Fertility: Preconception, IUI/IVF, Adoption, Surrogacy, Egg Freezing
- Variable length program depending on enrolled Product Line(s) delivering end-to-end care, including education and 24/7 care advocacy and support
- Fertility Product Lines can either be purchased individually or together as the Fertility Bundle
- A Participant may transition between one Fertility Product Line enrollment to another without incurring additional Per Enrollment Fees any time prior to the expiration of their then-current Fertility Product Line enrollment
- Educational content, quizzes, and coaching from women’s health and fertility experts to achieve health goals and explore paths to parenthood
- Career coaching around family planning in the workforce
- Referrals to Maven’s clinic and pharmacy partners (as applicable)
- Access to negotiated discounts and other benefits (e.g. priority scheduling) with Maven's clinic and pharmacy partners
- Available Product Lines:
- Preconception (12 months): Holistic approach to preconception care to promote overall health literacy for women of reproductive age, particularly around fertility and pregnancy, including support for mental health, nutrition, and career planning
- IUI/IVF (12 months): end-to-end care, including education, 24/7 care advocacy and support, and IVF and IUI support
- Adoption (12 months): end-to-end program supporting families throughout their adoption journey
- Surrogacy (12 months): end-to-end program supporting families throughout their surrogacy journey
- Egg Freezing (12 months): support and education, and, for self-pay customers of a Maven Partner Clinic, reduced costs on certain procedures within Maven Partner Clinic network, which may include free egg or embryo storage
Maven Wallet
- Maven Wallet can be activated by a Participant enrolled in a corresponding eligible Fertility Program Product Line (IUI/IVF, Adoption, Surrogacy, Egg Freezing), or Maternity Program Product Line.
- Wallet-enrolled Participants have access to Maven Wallet’s administrative reimbursement services through a platform that allows each Participant to aggregate the collection of required documentation for reimbursement of eligible expenses, in accordance with the Customer’s applicable employee benefit plan rules.
- Each Participant’s access to administrative reimbursement services shall commence upon the Participant’s activation of Maven Wallet in the platform and shall end upon the earlier of: a) when Participant reaches the Individual Reimbursement Maximum; b) 90 days after termination of this Agreement; or c) 90 days after termination of the Participant’s employment as reflected by their removal from the then-current Eligibility File or otherwise communicated by Customer in writing.
Maven Milk
- Breastmilk travel solution, providing nursing moms with an easy, reliable, and safe way to get their milk home to baby when traveling – both domestically and internationally.
- Each available Maven Milk product (as described below) has a separate Per Enrollment Fee. Customer shall be responsible for paying a Per Enrollment Fee for each product ordered by a Participant.
- The Maven Milk Products offered to Participants, which may be updated from time to time, are described at https://www.mavenclinic.com/maven-milk and currently include the following (each a “Maven Milk Product”):
- “Pump & Post Kit” Ground domestic shipping of cooler to Participant and expedited domestic shipping of cooler from Participant to baby at address specified by Participant. (Domestic only)
- “Pump & Carry Pack” Ground shipping of foam ice packs to Participant.
- “Pump & Check Kit” Ground shipping of foam ice packs and carry on cooler.
- Each of shipment of one Maven Milk Product is considered one enrollment in the Maven Milk Line (e.g., if a Customer Beneficiary is going on a multi-night business trip and orders three Pump & Post Kits, that would be considered three enrollments with three Per Enrollment Fees).
- The Participant’s access to services in the Maven Milk Line of the Program shall commence upon enrollment with Maven in accordance with this Agreement and shall end the earlier of: 24 months postpartum, when this Agreement expires or terminates, or when the Participant ceases to be an Eligible Customer Beneficiary (as indicated via the Eligibility File or otherwise by Customer in writing).
- Any incremental shipping costs, over what is covered in the Maven Milk as described above, will be billable to the Customer.
General Wellness
- 12-month offering; promote overall health literacy for women of reproductive age
- Career coaching around family planning in the workforce
- Educational content, quizzes, and coaching from women’s health and fertility experts to achieve health goals and explore paths to parenthood
Parenting & Pediatrics
- 12-month renewable offering supporting parents with children from 1 year old to preadolescence
- 24/7 pediatric care for emergency triage, episodic urgent care, second opinions, new parent anxiety, and more
- Daily guidance specific to each stage of childhood development to build stronger parent-child relationships and decrease behavioral issues
- Career coaching and concierge service to recommend in-person providers and help navigate childcare options, improving employee retention
Menopause
- Clinical care management to identify menopausal symptoms early and manage care throughout a member’s menopausal journey
- Specialized virtual care teams of providers focused on all stages of the menopause journey, available 24/7, to support individuals across all related issues including general menopause education, mood fluctuations, hot flashes, sleep issues, balancing work through menopause, and preventing chronic conditions
- Specialized referral lists to additional employer-sponsored resources and high-quality in-person providers
Attachment 1 to Appendix A
Maven Wallet will be enabled to support Participant reimbursements for eligible expenses pursuant to Customer’s applicable employee benefit plans.
- Customer shall complete Maven’s Wallet onboarding survey to confirm the design of its applicable benefit plan(s), including but not limited to member enrollment eligibility requirements, eligible expense types that Participants can have reimbursed, and tax treatment.
- Customer shall provide Maven an eligibility file on an agreed upon cadence, but at a minimum on a monthly basis.
- Each Wallet-enrolled Participant is responsible for timely submitting valid proofs of payment, itemized invoices for services, applicable bank account information, and any requested substantiation documentation to Maven and/or its designated agent.
- Customer and Maven will set up payment funding instructions during the implementation process. Customer shall be responsible for providing appropriate funds in advance for Maven Wallet reimbursements in order to complete the fund transfer to reimburse Participants in a timely fashion.
- Maven will collect expense reimbursement information from Wallet-enrolled Participants using its online platform.
- Maven will review expense reimbursement requests received from Wallet-enrolled Participants against the applicable benefit plan rules confirmed by the Customer, and will defer to the Customer for resolution of any questions that arise which are not clearly determined by the applicable benefit plan rules.
- Customer shall promptly respond to any questions from Maven regarding clarification of applicable benefit plan rules.
- Customer understands, acknowledges, and agrees that any Eligible Customer Beneficiary included in the Eligibility File will be considered eligible for Wallet reimbursements unless otherwise indicated by Customer in writing.
- Maven will compile receipts and expenses provided by Wallet-enrolled Participants and prepare aggregated Wallet expense reports and invoices (“Wallet Invoices”), which it will send to Customer on a weekly basis for any week in which a Wallet-enrolled Participant submits a reimbursement request.
- Customer shall confirm which Wallet Invoice expenses are approved to be reimbursed and ensure that sufficient funds are available for Maven or its designated agent to process approved reimbursement payments within 3 business days of Customer’s receipt of a Wallet Invoice.
- Maven will, at its discretion, either: (1) remit Customer-provided reimbursement funds to the applicable Participant’s bank account within 5 business days of Customer approving the reimbursement, provided that sufficient Customer funds have been made available for reimbursement and the applicable Participant has provided their bank account information; or (2) provide a report of approved reimbursement amounts, type, and tax treatment to facilitate Customer’s direct reimbursement of the applicable Participant(s) via its payroll system.
- Customer’s obligations under this Attachment shall continue for a minimum of 90 days beyond the termination of the Agreement, to facilitate Maven’s provision of its administrative reimbursement services for Participants enrolled in Wallet prior to termination of the Agreement who wish to submit reimbursement requests for eligible expenses incurred prior to the termination of the Agreement.
- Customer acknowledges and agrees that it is solely responsible for any communications to members about its employer-sponsored benefit plans, including any communications necessary to inform members of alternative claim submission procedures upon the termination or expiration of this Agreement.
- Customer, as the fiduciary, plan sponsor, and plan administrator of its employee benefit plan(s), acknowledges that it is fully responsible, and Maven shall have no liability, for the establishment, design and administration of Customer’s employee benefit plan(s) and all tax compliance and payroll reporting associated with any such employee benefit plan(s). Customer understands, acknowledges and agrees that: (A) Maven is not a plan sponsor, plan administrator or fiduciary with respect to any Customer plan supported by Maven Wallet; (B) Maven shall have no responsibility or liability with respect to any Customer plan supported by Maven Wallet; (C) to the extent that Maven provides any administrative or other services with respect to a Customer plan, (I) all such services shall be non-discretionary, ministerial services taken on behalf of Customer and at Customer’s specific direction, (II) Maven shall not take on any fiduciary or other obligations as a result of such services under the Employee Retirement Income Security Act of 1974, as amended (ERISA), or any other law, and (III) Customer shall remain solely responsible and liable for such services and any underlying obligations; (D) Customer will defend and hold harmless Maven and its owners, employees, officers, managers and directors from and against all liability, demands, damages, costs or expenses (including reasonable attorney’s fees) (collectively, “Liabilities”) arising from any claim, action or proceeding, in each case brought by a third party (each, a “Claim”) to the extent such Claim is related to any Customer plan supported by Maven Wallet, including, but not limited to, Liabilities that arise from any state or federal law, regulation, or judicial proceeding which may restrict or prohibit access to or the provision of certain counseling or medical care, including the provision of services or conduct in support of obtaining such counseling or medical care.
