PRODUCT LINE DESCRIPTIONS
All available Product Lines are described below. Specific Product Lines actually being purchased by Customer are identified on the Order Form’s Fee Schedule.
PRODUCT LINE
Maternity
- Up to 21-month end-to-end program supporting expecting parents during pregnancy and for 12 months after birth through the postpartum and return-to-work periods.
- Program includes Pregnancy, Partner, and Loss and Pregnancy Options tracks, and a Participant may move from one track to another within the same Maternity Product Line enrollment up to 12 months postpartum.
- Care advocacy and navigation support, including referrals to in-person OB-GYNs.
- Virtual practitioner network across 30+ specialties, such as OB-GYNs, Doulas, Career Coaches, Lactation Consultants, Mental Health Specialists, and Sleep Coaches.
- Content library of educational resources, wellness tips, and baby development tracking.
- Community forums for interactions with peers and virtual practitioners.
- Virtual group classes and videos, such as Breastfeeding 101, Infant CPR, and Newborn Care.
- Available product add-on: Breastmilk shipping program, Maven Milk, to support breastfeeding employees as they return to work (see description below).
Family Building: Fertility, Adoption, Surrogacy, Fertility Preservation
- 12 month renewable programs delivering end-to-end care, including education and 24/7 care advocacy and support.
- Educational content and coaching from reproductive health and fertility experts across 30+ specialties to achieve health goals and explore paths to parenthood.
- Community forums for interactions with peers and virtual practitioners.
- Virtual group classes and videos.
- Career coaching around family planning in the workforce.
- Referrals to Maven’s clinic and pharmacy partners (as applicable).
- Access to negotiated discounts with Maven's clinic and pharmacy partners.
- Available Product Lines:
- Fertility (12 months): Holistic approach fertility, promoting overall health literacy for women of reproductive age. End-to-end care from preconception through considering or undergoing fertility treatments, including education, 24/7 care advocacy and support, and support choosing a high-quality clinic.
- Adoption (12 months): Personalized coaching, communities, and care supporting families throughout their adoption journey.
- Surrogacy (12 months): Personalized coaching, communities, and care supporting families throughout their surrogacy journey.
- Fertility Preservation (12 months): Support and care for members through the considerations and process of fertility preservation, either for medically necessary care or elective care, including egg or sperm freezing.
Maven Wallet
- Maven Wallet is a reimbursement and 12-month renewable program that can be activated by a Participant enrolled in a corresponding eligible Maven Program Product Line. Enrollment fees are charged at the time a Participant enrolls and annually thereafter.
- Wallet-enrolled Participants have access to Maven Wallet’s administrative reimbursement services through a platform that allows each Participant to a) aggregate the collection of required documentation for reimbursement of eligible expenses, or b) to pay for covered services in accordance with the Customer’s applicable employee benefit plan rules.
- Each Participant’s access to administrative reimbursement services shall commence upon the Participant’s activation of Maven Wallet in the platform and shall end upon the earlier of: a) when Participant reaches the Individual Reimbursement Maximum; b) 90 days after termination of this Agreement; or c) 90 days after termination of the Participant’s employment as reflected by their removal from the then-current Eligibility File or otherwise communicated by Customer in writing.
- The Maven Card is a debit-like card that provides an alternative payment method for Wallet-enrolled Participants in lieu of using a Participant's personal form of payment prior to submitting a request for reimbursement through the Maven Wallet platform. The Maven Card can be used by Wallet-enrolled Participants who are part of certain Family Building tracks as identified by Maven. Participants electing to use Maven Card must substantiate all eligible expenses that are paid using the Maven Card and are subject to all terms and conditions of the Maven Wallet program.
- If Customer elects to offer the Maven Card, Wallet-enrolled Participants will be prompted to request a Maven Card through the Maven app and a card with activation instructions will be mailed to the Participant. Participants on a High Deductible Health Plan that contribute to a Health Savings Account will need to meet minimum statutory deductibles prior to being able to use the Maven Card for payment of eligible medical expenses. Use of Maven Card by Wallet-enrolled Participants is not mandatory; in lieu of using the Maven Card, Participants can seek reimbursement for eligible expenses by paying out of pocket and submitting a request for reimbursement through the Maven Wallet platform.
